When parents and children travel on national tours of Broadway shows, they should bring copies of the following documents with them, as state
departments of labor will request this information for obtaining work permits:

  1. The child's Social Security card.
  2. The child's birth certificate or passport.
  3. A guardianship statement (if child is being accompanied by someone other than a parent).
  4. A bank statement or passbook with the child's Coogan account information.
  5. A copy of the signed contract for the project.
  6. The parent's Social Security card.
  7. A parent's picture ID (i.e., driver's license, passport, government or
    military ID).
  8. A few most recent color photographs of your child.
